Register for MyWay Diabetes Somerset

This is a page for registering to access your diabetes-related medical records, which we call ‘patient access’. We will review your application and will send out further details on how to securely log in and access your information once we’ve reviewed your details.

Registration data fields include your name, address, email address, date of birth and your NHS number if you know it.

We will use these details to verify your identity and to find your records on NHS systems. We also use your details to send you our enrolment form and your user account login details.

Your information is sent to us securely and we will not share it with any unauthorised third parties.

Once you have full access to MyWay Diabetes Somerset it will allow you to access your diabetes information securely. The information is collected from:

Your GP Computer record
Your hospital clinic computer record
Other computer systems relevant to diabetes
